The form below gives us information about you and can be completed at your convenience. 

Please also complete a "New Pet Information" form for each pet you are bringing to our office. 

When using these forms save your form under your name then you may do one of the following:  

  • Fax to 614-792-9191
  • e-mail to office @ perimetervet.com 
  • or print it and bring it with you at the time of your drop-off or appointment 

If we have the forms ahead of time it can expedite your check-in.  If you do not wish to download the document or it does not work for you, there is a form you complete online in the "Online Forms" page of our web site and the information from this is sent directly to our office, though the format is not as ideal as the document below.
