We are very happy to welcome your pet to our veterinary hospital. 

The form below gives us very helpful information about your pet and can be completed at your convenience. 

Please complete a "New Pet Information" form for each pet you are bringing to our office. 

When using these forms save each form under your pet's name, then you may do one of the following:  

  • Fax to 614-792-9191
  • e-mail to office @ perimetervet.com 
  • or print it and bring it with you at the time of your pet's drop-off or appointment 

If we have the forms ahead of time it can expedite your check-in.  If you do not wish to download the document or it does not work for you, we will have you complete it at our office as this form is not available in the Online Forms section on our web site.  Thank you for taking the time to answer our questions.
