For information, please briefly describe the topic. For medication refills
please give us the name of the medication and your pet's name.
(Pets must have been examined at our office within the last year for
prescription medications or diets to be filled. If there is any reason
we cannot fill the prescription a member of our staff will contact you.)

Have medication or diet ready for me to pick up at the office
Mail me the medication ($5 postage & handling. A member of our staff will contact you with an invoice total)
